  H1N1 VACCINATIONS:

CONSENT FORMS  - Vaccination is voluntary. Students must have a consent form on file to receive vaccinations.  You can download a consent form from the PSD H1N1 flu information.  Return your completed form to school.

VACCINATION CLINIC - The vaccination clinic will be held after school on Thursday, December 10th . Only those children that have returned the signed consent form will be vaccinated .
